Unlimited Possibilities provides IHFS to individuals on a DD waiver who cannot live in their home without such services or who need outside support to remain in their home. Our services are provided to a person and their family members. The Unlimited Possibilities team collaborates with the individual(s) and their team to develop a customized schedule tailored to the individual(s)' person-centered goals, wants, and needs. This includes but is not limited to consistent volunteering opportunities, encouraging healthy hygiene habits, community outings, group programs, scheduled transportation needs, meal preparation, and assistance with household work side by side with the individual. We believe the key to an individual believing in their unlimited possibilities in life is a consistent, person-centered, customized IHFS schedule, developed personally by the individual, their family, and team.
